Nearly 30 years ago, Vaughan (1996) gave us a masterpiece of organization studies that explained why everyday organizational processes, cultures, work practices, and ways of thinking unwittingly combined to doom the space shuttle Challenger. Now Vaughn is back to explain why air travel is so amazingly safe.
